Pre-Employment Consent for Limited Queries of the FMSCA Drug & Alcohol Clearinghouse

I hereby provide consent to Consumers Energy to a conduct limited query of the Federal Motor Carrier Safety Administration (FMSCA) Commercial Driver's License Drug and Alcohol Clearinghouse (Clearinghouse) to determine whether drug or alcohol violation information about me exists in the Clearinghouse.

I understand that if the limited query conducted by Consumers Energy indicates that drug or alcohol violation information about me exists in the Clearinghouse, FMSCA will not disclose that information to Consumers Energy without first obtaining additional specific consent from me.
